 This is the page where you can find out what information we will need for each horse that is being seen by the vets of Amerty Equine. Because all of the records are in the process of being posted online, we do need as much information on your horse as possible.

FIRST TIME CLIENTS
Please submit the following information for ALL horses to be seen by Amerty Equine Vet & Farrier Services. This information is the SAME as the information required for the General 6 month checkup.

Your farm name: (Please list farm name even if you are a boarder.)
Your horses name, age, gender and breed.
If your horse has had any injuries or problems, PLEASE list that too.
Tell me about your horses temperament and something about him/her. Help me to get to know your horse! (List here any quipps that may assist me in seeing your horses. EX: Horse A likes to nip fingers. Horse B will kick. Horse C is a gentle boy..

For GENERAL  6 MONTH CHECKUPS  we need the following information for each horse.

If your horse has had any injuries or problems, PLEASE list that too.

For the MONTHLY CHECKUPS (Usually repeat customers use this. It is a great idea if your horses are heavilly shown. This is ALSO the same information needed for the General 6 month Checkup.)

For the following exams we are requesting specific information. Please do your best in providing this to us. It will be to yor benefit to provide as much as you know about the horse.

Pre-Purchase Exams: This exam comes highly recommended when purchasing an unknown horse. If you should choose to have us do a pre-purchase exam, PLEASE submit the following information.

What happened to the horse to make you think he/she is now lame?
